In-Home Follow up Consultation

Service Description

There are times when one visit may not be enough. That’s when a follow up appointments in your home or virtually is needed to address ongoing issues. Follow-up consults reassess current progress, complete a new feeding assessment, maternal assessment, weight check, and address any new or old concerns. I will also update the care plan if needed. Includes two weeks of support via phone, text or email to address basic questions and concern. Follow up appointments are for those who have already completed an initial consult. Appointments outside the 15 mile coverage area are subjected to an additional travel fee $10 per 5 miles. Venmo, paypal, cash and Credit card accepted. Super bill will be provided upon request that you may use to seek reimbursement from your insurance.  There will be a $50 cancelation fee for appointments canceled less then 24 hours in advance
